Well yeah, it hurts. It's going to hurt for a while. It'll be tender when you first get it.
There was a girl at my school who kissed her boyfriend and their liprings locked. She got frightened, pulled away, and a chunk of her lip came out.
So... they're not 100% safe.
As for infection, just make sure that if you get it done, your ring is made of precious metals (or however you call it). Apparently, it's easier to clean and not as harsh to your body and whatnot. And make sure the piercing dude is a professional and uses sterile equipment.
